WINSTON HILLS NETBALL CLUB
REGISTRATION INFORMATION
SENIORS - 2012 SEASON
Senior registrations are now being accepted. Please read the following and be aware that ALL conditions must be met to register for 2012.
Please send documentation and payment of $210 to PO Box 251, WINSTON HILLS 2153. A Senior Registration Day will also be held at Gooden Reserve, Baulkham Hills - 4th February 2012 1pm - 3pm.
1. Senior Registration is by TEAM only. You must delegate a co-ordinator of your team who can attend registration with your COMPLETED team form and player forms and Section 1 Theory Certificate and registration fees (WHNC - $210).
2. Each team must fill in a TEAM registration form.
4. Each team MUST have a nominated contact person - for all club correspondence.
5. Each team MUST nominate a minimum of 2 umpires capable of umpiring your requested grade or higher - 1 such umpire must have achieved a Gold or National badged qualification or is working towards that qualification within the current year of play. Umpires must be available to umpire in the final series.
6. Each player must have completed the Section 1 Theory Exam (see note below) and attach a copy of the certificate to the registration forms (unless a current pass is held) - for returning players, the theory pass is valid for 5 years.
Winston Hills Netball Club Committee
(Section 1 Theory Exam - available 24/7 on the Netball Australia Website - www.netball.asn.au - click on the "umpire's exam centre". You can attempt it as many times as you need to pass it - when you pass, simply print a copy of your exam result and attach it to your registration forms. They are valid for six years)
PLEASE check that you have completed ALL necessary forms. Registration will not proceed unless all forms are correctly filled out and submitted.

- Team Rego Form and Senior Registration Forms
- Copy of Section 1 Theory certificate
- BHSNA Registration Form
- WHSC Application Form or Membership Number
- Passport Photo (if needed)
